Trumbull County Combined Health District to host 4 COVID-19 vaccination clinics this week

The Trumbull County Combined Health District will host four COVID-19 vaccination clinics this week at various locations throughout the county.

The Moderna, Pfizer and Johnson & Johnson vaccines will be available.

Clinics are scheduled for:

  • 5 to 7 p.m. Monday at Southington Town Hall, 3419 Ohio Highway 534, Southington;
  • 4:30 to 7 p.m. Tuesday at Trumbull County Combined Health District, 176 Chestnut Ave. NE, Warren;
  • 5 to 7 p.m. Wednesday at Trumbull Community Action Program, 1230 Palmyra Road SW, Warren;
  • 5 to 7 p.m. Thursday at Girard Public Library, 105 E. Prospect St., Girard.

Walk-ins will be accepted at the clinics, but appointments can be scheduled here. If you schedule an appointment, bring your confirmation email or text with you to the clinic. To speed up the check-in process, recipients are encouraged to visit the health district’s website to download, print and complete the COVID-19 Vaccine Administration Record form and bring it to the clinic. If you are receiving your additional or booster dose, bring your vaccine card with you to your appointment.

If you are under an Ohio managed care Medicaid plan, you may be eligible to receive a gift card through the mail for a Moderna or Pfizer booster. Information will be provided at the clinic.

Ages of eligibility are 12 and older for the Pfizer vaccine, 5 to 11 for the Pfizer pediatric vaccine and 18 and older for the Moderna and Johnson & Johnson vaccines.
